NC2 is excited to announce a new partnership with the Center to Advance Palliative Care (CAPC). Through this agreement, NC2 will be granting CAPC membership to as many as three (3) of our Organizational Partners in 2018.
Those who are awarded grants will have 100% of their CAPC membership dues paid by NC2 for the first year, a $3500-$6000 value! Please review full details below prior to applying.

What’s Included with CAPC Membership?
Your organization will become part of a vibrant collaborative network of health care professionals working to strengthen the palliative care skills and knowledge of all clinicians and to ensure access to quality palliative care in all settings across the continuum of care. One CAPC membership provides the entire staff of each member entity with access to the clinical and operational training, tools, technical assistance, and other expert resources.

What Are the Expectations of Grantees?
Grantees must submit a grant application before May 25, 2018. Those selected must also complete a pre-assessment prior to June 15, 2018 and semi-annual assessments during the membership year so that NC2 can gauge the impact of this CAPC partnership as well as the benefit to your organization, staff and palliative care activities. Finally, grantees shall give NC2 permission to verify with CAPC utilization of site resources.
